The vaccination clinic, operated by the George Mason University School of Nursing and the Mason and Partner (MAP) Clinics, have vaccinated more than 22,305 residents since the vaccine became available in January 2021, predominantly serving under-represented and uninsured populations.  </span></span></span></span></span></span></p> <p> </p> <p><span><span><span><span><span><span>Wexton, Roem, and Rishell thanked the nurses and staff running the vaccination site and spoke with clinic coordinator Bridget Jennison about George Mason and the MAP Clinics’ vaccination efforts. “The MAP Clinics are pleased to help greater Prince William fight COVID by offering vaccination clinics and partnering with the Prince William Health Department to reach as many people as possible,” said Jennison. </span></span></span></span></span></span></p> <p> </p> <p><span><span><span><span><span><span>More than 1,000 doses were administered the day of the officials’ visit, though Jennison explained that the number of vaccinations was slowing down. Jennison indicated that the next wave of vaccinations would be for 12-15 year-olds (approved by the Food and Drug Administration for emergency use on May 10 and the Centers for Disease Control and Prevention on May 12) and those who may be harder to reach because of hesitancy or other factors. Jennison, Wexton, Roem, and Rishell discussed opportunities to work together to increase vaccination rates in the Prince William community and expressed optimism for continued and successful progress.</span></span></span></span></span></span></p> <p> </p> <p>“I am very impressed with the MAP Clinic’s commitment to the greater Prince William community and am thankful for their help vaccinating so many residents. Virginia is currently one of only 15 states that does not have an accredited college of public health. Many early childhood education centers, including ImagiNation Learning Center, have remained open throughout the pandemic, providing continuity of care and allowing families who rely on these providers to return to work. Early childhood educators are part of Phase 1b for Virginia’s COVID-19 Vaccination plan– however many in this population were not aware they were eligible for a vaccine.</span></span></span></p> <p> </p> <p><span><span><span>To help raise awareness and increase vaccination rates in Greater Prince William, Mason and Partner (MAP) Clinics, Smart Beginnings Greater Prince William (SBGPW), and the Prince William Health District (PWHD) partnered to schedule more than 1,400 appointments with early childhood educators</span><span> over the course of seven days. On the day of the First Lady’s visit, more than 500 first doses and 150 second doses were administered.</span></span></span></p> <p> </p> <p><span><span><span>“In just one week, we have contacted more than 350 early childhood education centers</span><span> to let them know that they fall into Phase 1b <em>and</em> are eligible for the COVID vaccine.  Organizing these clinics for those who educate our youngest children has been the most rewarding work I’ve done since COVID began. The dramatic shortage of masks and other vital personal protective equipment (PPE) coupled with the stay-at-home order limit providers’ ability to deliver medical care, particularly for underserved populations who may not have insurance or a primary care provider.</p> <p>However, despite all these changes, patients can still rely on the <a href="https://chhs.gmu.edu/mapclinics">Mason and Partners (MAP) Clinics</a> (a network of 10 no-cost bridge health care clinics supported by the College of Health and Human Services at George Mason University) for the care they need thanks to the rapid deployment of expanded telehealth capabilities. By expanding the MAP Clinic’s telehealth capabilities, the MAP Clinics can now directly combat the pandemic by screening for COVID-19 while helping their patients battle chronic conditions, treat substance use disorders, and address behavioral health issues such as anxiety and stress. </p> <p>The nurse-managed MAP Clinics rapidly deployed HIPAA-compliant telehealth units to meet the demand of underserved communities in our region where there remains limited access to COVID-19 screening. MAP Clinic co-directors have chosen to focus more resources in Prince William County where seven of the 10 clinics are located and significant unmet demand exists among very vulnerable populations: the uninsured, those experiencing homelessness, and those being released from jail and possibly at higher risk of opioid overdose. While Fairfax County is also home to three of 10 MAP Clinics, Fairfax has existing infrastructure for testing and treatment.</p> <p>Existing patients from all 10 of the MAP clinics were transitioned to telehealth visits, and the clinic has coordinated care for 700 patients in the past two weeks. Each unit consists of a tablet pre-loaded with HIPAA-compliant apps, consent forms, and teaching packets to help the end-users at each site effectively screen for COVID-19 symptoms. “The MAP Clinic telehealth initiative is the best example of teamwork and partnership I’ve ever seen. Our students and faculty from across the College are working with community partners to serve our most vulnerable patients even we cannot physically be with them,” says Dr. Rebecca Sutter, co-director of the MAP Clinics and associate professor of nursing at Mason.</p> <p>The expanded telehealth model and revised protocols allow MAP Clinic staff to use their limited PPE to treat the highest-risk patients. The MAP Clinics continue to serve their existing patients, walk-ins, and those referred by the Prince William Health Department in-person two days a week at the Manassas Park MAP clinic. In addition to fighting COVID-19 head-on, this MAP Clinic also continues to provide on-going care including <a href="https://chhs.gmu.edu/news/583601">suboxone treatment</a> and chronic disease panels for diabetes, depression, anxiety, and hypertension. Telehealth visits, combined with in-person care, allows nurse practitioners, registered nurses, and social workers to connect with patients regularly to see how their medications are working and provide support – especially during this stressful time. In addition to providing life-saving care for patients, the MAP Clinics provide students pursuing degrees in health informatics, nursing, and social work with experience in serving rural populations and in deploying a fully operational telehealth unit.</p> <p>“It has been amazing to see the MAP clinic team coordinate the transition to telehealth quickly and responsibly while also giving students the opportunity to continue their clinical learning during this time,” says Caroline Sutter, co-director of the MAP Clinics and associate professor of nursing at Mason.</p> <p>The MAP clinics are working toward point-of-care testing, which will provide results in 45 minutes. “This will be a game-changer for our patients,” says Rebecca Sutter. “Right now we can only do presumptive positive treatment, given that it can take 10 days to get test results.”</p> <p>The Washington, D.C. region is considered a “hot spot” for future peaks in COVID-19 cases, and the MAP Clinics are further expanding telehealth and training capabilities to meet expected surge in demand for testing and care. The project will train medical professionals on how to appropriately prescribe opioids, screen for and identify the risk of opioid use disorder, and deliver treatment.</p> <p>The telehealth initiative will be carried out through <a href="https://chhs.gmu.edu/mapclinics">Mason and Partners Clinics</a> and the new <a href="https://chhs.gmu.edu/PopulationHealth">Population Health Center,</a> which will serve as hub-sites for the telehealth training. The grant will enable Mason to connect medical professionals with low-income residents of Virginia and West Virginia who have limited access to specialized care, yet are in high-risk areas for opioid use disorder and chronic conditions. Each unit consists of a tablet pre-loaded with HIPAA-compliant apps, consent forms, and teaching packets to help the end-users at each site effectively screen for COVID-19 symptoms. The grant allows MAP Clinics to partner with rural organizations—like the mobile care provider The Health Wagon—to serve patients despite financial and transportation barriers and to reduce in person visits that can spread COVID-19 in rural parts of the Commonwealth.</p> <p>In 2017, <a href="https://www.drugabuse.gov/opioid-summaries-by-state/west-virginia-opioid-summary">West Virginia had the highest of opioid-involved overdose deaths</a> at a rate of 49.6 deaths per 100,000 people. In 2018, <a href="http://www.vdh.virginia.gov/opioid-data/">an average of three Virginia residents died of an opioid overdose daily</a>. In her first year, the congresswoman has successfully achieved a major legislative victory when the House passed her bill, H.R. 3153, the Expanding Findings for Federal Opioid Research and Treatment (EFFORT) Act, which directs the National Science Foundation to support merit-reviewed and competitively awarded research on the science of opioid addiction.  </p> <p>Wexton visited the Manassas Park Mason and Partners (MAP) Clinic to see first-hand how this unique bridge Medication Assisted Treatment Mobile Clinic (MAT MOBILE), funded in part by Northern Virginia Health Foundation and managed by George Mason faculty and students, have helped members of her congressional district fight opioid addiction.</p> <p>As part of the MAT program, the MAP Clinic in Manassas Park delivers free suboxone treatment using a peer recovery specialist model one day a week. To date the Clinic has served more than 40 patients and serves 7 to 14 patients each day the service is offered.  </p> <p>The Clinic works in partnership with the criminal justice system to offer patients who are court-involved a safe transition from jail back into the community.  “The first few days after being released from jail is a high-risk time for overdose,” explains Rebecca Sutter, director of the clinic, “Which is why we work with the Adult Detention Center, Community Service Board, and local probation to connect our patients to a high-touch system of support.”</p> <p>“It’s very impressive,” said Wexton. “I’m so glad to have Mason and Partners in the community.” Wexton said as she toured the facility and met the staff, including Jess Stoler, the licensed practical counselor and Francisco Morin, a Mason DNP student and lead educator at the Clinic.</p> <p>The MAP Clinic model is based on an academic-practice partnership model to help translate best evidence into practice at scale for the largest possible impact in the community. With the majority of care transitioning from hospitals and emergency departments to the community, facilities such as the MAP Clinic are increasingly important to keeping populations healthy.  The MAP Clinic is funded through Health Resources and Services Administration (HRSA) grants, a program that has sustained significant budget cuts recently. Thirty-five students and faculty staff the Clinic each day, delivering a range of services throughout the week. She has developed, implemented and evaluated a unique model of community healthcare access and navigation called The Bridge Care Model. This model of care uses a systems approach to improve care and promote collaboration between public health, public safety and health to ensure that evidence based treatment is available for at risk and marginalized individuals, families, communities and populations. She uses the academic nurse managed clinics, which are strategically located in medically underserved areas (MUA) across the Northern Virginia region, to provide services to underserved and vulnerable populations and conducts translational research on evidence-based care for patients with limited resources. She has expanded this model of care to include criminally involved at risk individuals being released into the community using a comprehensive and coordinated approach to connect them to needed health and social services using care navigation and peer recovery support programs. Prior to joining the School of Nursing, Dugger joined George Mason University in 2015 as the Director of Field Education in the Department of Social Work. In her role, Dugger led initiatives of experiential learning opportunities through internships with local organizations for both undergraduate and graduate social work students. The Field Education Office is a liaison between the 350 graduate and undergrad students and more than 250 active partners. Now with the Mason and Partners Clinics, Dugger serves as the clinical preceptor for social work students and leads initiatives surrounding social work and clinical counseling services for all patients at the MAP Clinics. She received her MSW from the University of Kansas and practiced community social work for 13 years. She served in the nonprofit sector of children and families, child welfare, and developmental disabilities.
